  1. On this page you will find a PDF version of our job application. You can either fill it out and submit it online, or you can print and mail the application to us at:

    or, if you would rather hand deliver it, we recently
    moved to the North Campus on Frazee Street,
    directly North of the Clinic.

  2. You must fully and accurately complete the Application for employment. Incomplete applications will not be considered. St. Mary's Innovis Health may use the information given in the application to investigate the applicant's previous employment and background.
  3. The Application for Employment will be considered inactive after 2 years. If you wish to be considered after that time, you must complete a new Application for Employment.
  4. If you are hired, proof of citizenship or immigration status will be required to verify your lawful right to work in the United States.
