You can request that Innovis Health release medical record information to another party by completing the Release of Information form below.

All release forms must be mailed or faxed to the Medical Records department. Federal law does now allow requests via email or phone. Requests usually take between 5 to 15 working days for us to provide the information you request. You will receive a call when your records are ready to be picked up at St. Mary's Innovis Health.
